Gather AI - Allow Variables in Custom Questions

We use the Custom Questions to gauge client comfort in using Safe Send and electronic delivery. We still use a lot of paper in my office, and when the custom questionnaire is completed we like to print it out and put it in our client folders to reflect their preferences. Unfortunately if we're just printing those questions there is way to identify the client.

It would be useful to have an option to include the client name as a header / footer by default OR allow variables in the questions where we could print the client name(s) that way.

An alternative would be a downloadable CSV that shows the client answers for each question in a single CSV file.
